Privacy Rights Request

Use this form to request access to your Personal Information, obtain specific pieces or a portable copy, correct inaccurate information, request deletion, or withdraw applicable consent.

Requests to opt out of sale, sharing, targeted advertising, or qualifying profiling should be submitted through our Privacy Choices form.

To appeal a decision on a Colorado privacy request, use our Colorado Privacy Appeal form.

We may need to verify your identity before completing this request. Please do not enter passwords, Social Security numbers, complete payment-card numbers, or copies of government identification unless we later request information through a secure verification process.

Access and portability details

We do not send specific pieces through ordinary unencrypted email. We use an authenticated portal or an expiring secure-download link instead.

After submission, Pidoxa may email you with reasonable verification steps. The verification required will depend on the nature and sensitivity of the request. Information collected for verification will be used only to verify and process the request, prevent fraud, maintain required records, and protect security.
